On-Site Case Management: Medical Case Management

Future Care's Field Case Management offers clients a unique service. Future Care provides individualized, on-site case management, through our national network. When Telephonic Case Management alone is not sufficient to determine the best possible medical case management plan, Field Case Management takes the evaluative process out of the office and into the field, to the individual's home, workplace, and health care treatment facility, all in the service of a singular goal; the expeditious and medically appropriate resolution of a claim.

Future Care enhances the On Site Case Management process by:

• On-site consultation with employers to explore modified-duty or early return-to-work options
• Personalized supervision of the transition from hospital to rehabilitation center, or from rehab to home care, to assure provision of services and consistency of care
• Professional oversight of scheduling and services provision
